You can save up to 20 minutes in our office by filling out the patient registration and health history forms. Simply print each form (PDF Document), fill it out completely and email, fax or bring to the office with you. If you do not have the Free Adobe Acrobat Reader, click here.
Email or fax will further expedite the process by allowing us to verify your insurance coverage and information before your arrival.
